Health Forms

The School Code of Illinois, Section 27-8 (Public law 95-422), has been revised and now states that every child shall have a physical examination upon entrance to kindergarten and at the beginning of 6th and 9th grades. Failure to comply with this state law is cause for exclusion of you child from school.  To meet this requirement, the exam form must be signed by a doctor and completed after February 1st, of the current school year.  Children whose completed physical exam forms have not been filed with the school office may be excluded from school until this requirement is met.

 


A dental exam is required for all kindergarten, second, and sixth grade students. The dental examination form is to be completed and on file by May 15 of the school year.  A dental waiver is available and can be completed if applicable.


 

An eye examination is required for any child enrolling in kindergarten or for new students to an Illinois school. Each student shall present proof of being examined by a licensed physician before October 15th of the school year.


 

All required forms are located on this website.

Health records transferred from another school will be accepted providing the physical and dental records are complete, and the examinations were given after February 1st of the current year.

Because physicians and dentists are rushed to complete exams in the summer, we suggest you make an early appointment.  Please don't delay in completing the required exams.  The forms can be mailed or brought to the school offices as soon as possible.

Attendance:

Please call to the school office or attendance line early in the morning when you know your son or daughter will be absent from school. If your child will be absent for an extended time, please send a note to school stating the reason for the absence and the days he or she will be out of school.


Guidelines for Keeping your Child Home from School:

Please keep your child home if he/she has a fever or abdominal pain with vomiting or diarrhea. Your child may return to school after being symptom free for one full school day. If you have any questions about sending your child to school, please feel free to call the nurse's office.


Medications at School:

A doctor's order is necessary to administer any medications at school including all over-the-counter medications. The medication needs to be brought to school in the original label container along with the Administration of Medications form which is located on this website.

New Spirit Wear Program

New Spirit Wear Program at Madison, Edison and Lincoln:

We are excited to pilot a new spirit wear program.  Attached are digital order forms for Lincoln Jr. High, Edison & Madison schools.

We will be working with a fulfillment house so the PTA won’t carry inventory moving forward.

This will be the only order for the remaining school year, so please be sure to place your order for spring field trips, field days, school spirit days or if your child is moving to Edison/Lincoln to ensure your child is outfitted for the start of the next school year.

Next year we will offer two opportunities to order during the school year.

In order to keep your costs down, all sales are final.  No refunds or exchanges.  One sample of each item will be available in the school offices before March 23rd.

Skokie Voice Forum - 2/23/11

Skokie Voice Forum - Skokie Voice will be hosting a community forum next week on Wednesday, Feb. 23 from 7-9:00 p.m at Oakton Community Center.

The featured speaker will be Mayor Van Dusen who will give a brief statement of the vision of Skokie and then answer questions from residents that were submitted in advance of the forum. In addition, there will also be a live Q/A where Mayor Van Dusen along with Asst. Village manager John Lockerby will answer questions from the audience on issues that affect the community. We hope that residents will come and share their vision with the Mayor and get answers to their questions.

The Skokie Voice Community Safety, Housing and Economic Development Committee Chairs will also give updates on what they have been working on in the community.

Skokie Voice would like to see hundreds in attendance again (like the June 2010 Town Hall meeting) so publicizing this community event is very important. Skokie Voice is very grateful to the many school districts who have been instrumental in helpful us to reach the residents across the community by posting the Skokie Voice flier on their school district sites as well as posting on electronic signs.

FUND69 Spring Fling 2011

Where can you play Wii games without having to compete with your kids?  Where can you buy a $5.00 raffle ticket to win a Toshiba notebook computer or a $100 Visa gift card?  Where can you bid on items like a Skokie Park District birthday package, autographed Cubs and White Sox photographs, a hotel getaway or even the chance to be superintendent for half a day?  All while enjoying a great dinner and cocktails AND while helping the students of district 69?  FUND69’s “Wii Without Them” of course!!!

The reservation deadline has been extended to Friday, February 18.  Don’t miss out on the fun!!  The cost is only $35/person for a great evening of fun for a great cause.

Raffle tickets are now on sale, the winner does not need to be present to win.  They are available in all the schools (contact Debbie Rizki at Madison, Emily Schmidt at Edison and peter McCall at Lincoln), from any FUND69 board member, or by emailing Annie Uchtman, "annie at mollyu.com".  Tickets are $5 each or 5 for $20.
