Information for Families New to District 69 and Edison School

The following documents need to be presented by the parent(s) for verification at the time of registration. Please note that acceptable proof of residency from both Category A and B is required. All requirements must be met before the student can start school.
 
      Birth Certificate  


    An original certified copy of the Birth Certificate is required to enroll the student in District 69. A certified copy will have the seal of the county on the certificate.

    If your child was born in Cook County, a copy can be obtained at the Cook County 2nd Municipal Court House, 5600 Old Orchard Rd in Skokie (847.470.7500, press 3) or at participating currency exchanges. If not, then contact the county of birth for information on obtaining a birth certificate.

    -If the child's birth certifcate is in a language other than English you must provide and official translation.

    -Passport or visa wll be accepted if child was not born in the US.

    -Hospital and/or baptism certificates are not accepted.

    Most recent real estate tax bill and proof of payment.
    Signed and dated lease, w/ landlords name, phone
    number and proof of payment
    Closing statement from purchase of residence

    In lieu of a lease, a notarized letter of residence
    from landlord

    All documents must be original, if you do not have any
    of the above affidavits must be obtained from the school
    office.
 
     Category B (Provide Two)  
     
    Driver's License or State ID w/District 69 address
    Current gas, electric or cable bill (Only one is accepted)
    Public Aid card
    Voter registration car or application for voter
    registration card
    Home/apartment insurance certificate
    Vehicle registration - State of Illinois w/ District 69 address
 
      Other Helpful Information  
     
    All students enrolling from another Illinois school must have had a physical in the last 12 months. Those enrolling from outside of Illinois must have had one in the last 6 months. All immunizations must be up to date, in accordance with the State of Illinois guidelines. A copy of the certificate of child health form can be found here.

    Please remember to bring the Illinois in Good Standing Transfer from your child's previous school if you are transfering from an Illinois school) as well as any medical records.

    In addition , bring the contact information from the previous school including the exact name, street address and zip code.
